DOCOMO may provide the information listed below (the “Information to be Provided”) concerning forged or falsified certificates presented by you or an applicant for subscription, to mobile telecommunications carriers that provide mobile telecommunications services.

- Certificate information (type of certificate, certificate number, and issuing authority)
- Application date
- Method or manner of forgery
- Results of contract screening and related information
[Purpose of Use by the Recipient]
Personal information contained in forged or falsified certificates will be exchanged among mobile telecommunications carriers and used in the screening process at the time of accepting applications for subscription, in order to prevent fraudulent contracts and to protect fairness and the interests of all users.
Please note that, depending on the results of such screening at the time of application, we may be unable to accept your application.
