Application Procedure

The items required for application for the Hearty Discount and the application service counters are as follows.

Items Required

Application by a customer with a DOCOMO line subscription

One of the following documents must be presented: Government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo), government-issued identification booklet for the mentally challenged (Ryoiku Techo), government-issued welfare booklet for the mentally ill (Seishin Shogaisha Hoken Fukushi Techo), government-issued specified disease treatment recipient certificate (Tokutei Shikkan Iryo Jyukyushasho), government-issued specified disease registrant certificate (Tokutei Shikkan Torokushasho), and specified intractable disease medical expense recipient certificate (Tokutei Iryohi [Shitei Nanbyo] Jyukyushasho)

New windowMIRAIRO ID, a disability certificate smartphone app, is also accepted as valid documentation at docomo Shops, retailers, or general distributors.

Applying by post

Send the Hearty Discount application form and a copy of the subscriber's booklet or certificate.

Applying at the same time as applying for a new subscription for a mobile phone

The following documents are required.

  • An original copy of the subscriber's government-issued identification booklet for the physically challenged (Shintai Shogaisha Techo), government-issued identification booklet for the mentally challenged (Ryoiku Techo), government-issued welfare booklet for the mentally ill (Seishin Shogaisha Hoken Fukushi Techo), government-issued specified disease treatment recipient certificate (Tokutei Shikkan Iryo Jyukyushasho), government-issued specified disease registrant certificate (Tokutei Shikkan Torokushasho), or specified intractable disease medical expense recipient certificate (Tokutei Iryohi [Shitei Nanbyo] Jyukyushasho).
    • If the address shown on the document is different from the current address, one of the additional documents below must be presented.
  • The subscriber's credit card or bank card
    • If the subscriber does not have a credit card or a bank card, the subscriber's "bankbook plus seal" and one of the additional documents below must be presented.

An utility receipt or the Certificate of Residence (Juminhyo) without Individual Number (social security and tax number) printed on it

  • The additional documents must be issued within the last three months with the subscriber's current address and limited to one in the name of the subscriber.
  • Only utility receipts for electricity, gas, water, etc. bearing a receipt stamp or automatic payment notices showing the date of automatic payment are accepted.

If the subscriber is a minor

A letter of consent*1 bearing a legal parent or guardian's signature and seal, and documents verifying the identity of the legal parent or guardian must be presented in addition to the booklet or certificate of the subscriber.

Application by a representative is acceptable if the subscriber is unable to apply for the service

The following documents are required in addition to the booklet or certificate of the subscriber.

  • Letter of Attorney with the subscriber's signature or written name and seal*2
  • A document to verify identity of representative
    (An original copy of representative's Japanese driver's license or Individual Number Card (My Number Card)*3)
  1. A letter of consent or letter of attorney can be downloaded from Letter of Consent/Letter of Attorney Downloads.
  2. The identification document must be valid and show the current address.
  • In order to apply for Hearty Discount at the same time as a new mobile phone subscription, the applying representative must be a member of the subscriber's family or a legal representative.
  • When a legal representative applies, a letter of consent from the legal representative, identification document of the legal representative, and a document to verify that the person is a legal representative are required in addition to the booklet or certificate of the subscriber.

You can apply by PC, smartphone, docomo Feature Phone, phone, or at a docomo Shop/d garden.

  • If you apply by phone, PC, Smartphone, or docomo Feature Phone, you will be sent an application form by post to fill in and then send back.
